DISCHARGE SUMMARIES: DECANNULATION

POST DECANNULATION TRACHEOSTOMY SITE CARES:


The tracheostomy site was sealed with a clear occlusive dressing covered with gauze and sealed with foam tape. Every day the trach site needs to be checked for signs and symptoms of infection which include, but are not limited to, swelling, redness, discharge, and/or fever. Check the site by lifting the foam tape and gauze. Do not remove the clear occlusive tape underneath. Check for air leak: have patient cough. If an air bubble forms under the clear occlusive dressing, the trach site is still open and the patient should replace the gauze and foam tape. When no air bubble is exhibited by cough, then the trach site is closed and all dressings can be removed.
