A Buzz In The Ear

Question:  A 32 year old female is referred to you for pulsatile tinnitus.  Otoscopic exam reveals a reddish hue behind the right tympanic membrane.  You notice that her voice is slightly hoarse which she remarks has been present for 6 months.  Oral cavity examination reveals fasciculations of the right hemi tongue.  What is your presumptive diagnosis?  [Answer will be posted with next week's new question]
